Understanding Spinal Stenosis
Spinal stenosis is a common and potentially crippling condition affecting most of us to some extent as we get older. It can occur anywhere including cervical stenosis or spinal lumbar stenosis. Although there are many types and causes of spinal stenosis, the most common occurs over the age of 60, affecting women slightly more than men.
Spinal stenosis is also called the great masquerader since it can mimic several other conditions such as peripheral vascular disease, neuropathy, large joint problems, or central nervous system disorders.
The most common initial step in treatment of spinal stenosis is physical therapy. Emphasis in physical therapy is placed on strengthening the muscles of the back, stretching these muscles, and improving posture. By better supporting the spine, symptoms of nerve compression are often improved.

A recent study compared two physical therapy programs for patients with spinal lumbar stenosis. Fifty-eight patients with spinal lumbar stenosis were put in one of two 6-week physical therapy programs. One program included manual physical therapy, treadmill walking, and exercise, while the other included lumbar flexion exercises, a treadmill walking program, and ultrasound.
A greater proportion of patients in the manual physical therapy, exercise, and walking group reported recovery at 6 weeks compared with the flexion exercise and walking group. At 1 year, 62% of the manual therapy still reported a good recovery. Improvements in function, pain, satisfaction, and walking tests favored the manual physical therapy and exercise group at all follow-up visits.
Patients with lumbar spinal stenosis can benefit greatly from physical therapy, especially manual physical therapy.
